Wishes about housing
Wishes about housing you write in ordinary words. You want to stay at home as long as possible, or you want to go to a particular nursing home when it is no longer possible, or you do not yet have a choice. All three are allowed. Name the home or the institution if you already know it, and leave the line open if you do not know it. Also write who may think along in a conversation with the care providers: a name and a telephone number. That person is someone to talk with whom you name now. The sentence does not establish that that person decides for you in a formal sense. It is a wish about how you want to live and whom you want with you when there is a conversation. The sentence has no more authority than that.
This wish is not a mentorship (mentorschap) and not a levenstestament. A mentorship (mentorschap) and a levenstestament are documents with their own form, outside this note. A field in the life file does not put them in place and does not end them. The levenstestament, if you have one, works during life when you yourself cannot act, and it stops at death. Where the original of that document is, you note separately. Here it stays with the wish about home or an institution. Do not write that the person named may sell the home or act as guardian. That reads as an authority, and this note does not give it. An empty line is better than a sentence that looks like a deed.
If there is a written wish with the GP about care or housing, you only point to it. You leave the text itself with the doctor. Treatment wishes belong there, during life, not as an instruction in this file. You point to the donor register in the same way, without copying out the registration. Ervazo stores what you fill in here and does not send it to a doctor and not to a notary. If your wish changes, you rewrite the sentence and you add the date. An old preference for an institution you no longer want, you remove. The family that later reads only the old sentence cannot know that you have changed. Once a year you read this paragraph again.
A life file is a note for later. It is not a will, not a medical instruction and not advice. The wish about housing says what you want now and who may think along. It appoints no mentorship (mentorschap), no heir and no executor. An executor and an heir belong in a notarial will. A codicil has its own, strictly handwritten form and is not this wish. Ervazo does not send the note to a doctor or a notary. Whoever reads the sentence can take it to a conversation. The sentence does not decide that conversation and it does not extend a power of attorney beyond death.
